Upper face neurotoxin treatment includes the forehead, frown lines (glabellar complex), and crow's feet (bilateral lateral orbicularis oculi muscles) not to exceed the FDA approved 40-64 units for this area. Dysport, Daxxify, Xeomin, and Jeuveau are included in the unlimited upper face Tox treatment. High-dose Daxxify is excluded. You may schedule your Tox treatments every 3-5 months with this membership.
If you want to cancel your membership, you must provide YO with a sixty (60) day's written notice via email. Your membership will continue for those sixty (60) days, including the monthly payment. If you wish to terminate earlier than six (6) months, you are responsible to pay for the full six (6) months.
